This can be done in both HOTD4 story (6 A-S Grades) and in HOTD4 SP story(2 A-S Grades)
SP mode is definitely easier to get, obviously

Don't make it hard for yourself and play on very easy, go for head shots and attempt to get all hidden score items, however don't lose it on accuracy, finally you don't have to get the good ending to get an A or S, i got a bad ending and ended with 2 A's first time

People just do HOTD4 SP i got it the 1st time i played HOTD4 SP

To get a trophy advise enable HOD4 SP and the level of "Very Hard".

In it you will be Dowa more game points and the accuracy you will have no less than 70%

Also, you should eventually be ranked AA
