what do they mean with 'clear' should we kill all creatures inside 5 dungeons?

I think dungeons are like sidequests. If you complete them than you've cleared them. But offcourse, those are just my thoughts. We have to wait and see it on 11.11.11 to be sure. :)

meaning all creatures and enemies slain and collectibles found = cleared

I remember reading somewhere that this game will have areas that are completely done with (I don't remember it being specified how though) the area on the map will be flagged as complete. 5 dungeons with a flag, gets you the trophy maybe?

I think you're correct. I've been reading a forum of 360 pirates(lucky bastards have been playing Skyrim for several days by now) to learn more about the game and they say that 'cleared' is indeed visually marked on the map and there's a confirmation message. So you won't have to go worrying if you have missed a rat deep in the dungeon or not. But that's 50, not 5, so they'd better make some interesting dungeons or this may turn out to be a chre. It's what I hated the most in Oblivion/Fallout 3: you delve deep into a dungeon and spend some hours exploring every nook and cranny only to get over-encumbered with useless junk. That's not really specific to Bethesda games though, almost every RPG nowadays has a tons of stuff that is almost useless but still has some value so you don't want

They are marked as cleared. You don't actually have to kill everything in the cave. From my experience so far, every cave has one npc at the end that's tougher than the rest, like a boss. It seems that killing that guy marks the dungeon as "cleared".

If you are close to finishing this then try doing caves you've discovered and havnt cleared. They are generally shorter and smaller than forts and ruins.

I think spots with mammoth and giants also count. As they is only 1 or 2 mammoth and 1 or 2 giants the "dungeons" can be cleaned really quickly.

hat counts as a dungeon? obviously caves, but just in general anything else that can say "cleared"? such as bandit towers and such?

Dragons guarding Word walls count as dungeons too. Places like Sherepoint.
