Platinum/PSN Trophy Card Request Thread

First of all, thanks to spratters for running this thread when I was too busy, for almost a year.

The rules for requesting a Platinum/PSN Card/Avatar:

• You must fill out the form (see posts below), so we can deliver the card you want. (read the rest of the rules first).
• You will need to have 100 non-spam posts and have been a member for over two months before requesting a card. If you don't meet the requirements, stick around, be active, and then request one.
• You must sync your trophies to PSN, and use YourGamerCards, US PSN Card, Playfire or similiar.
  Please make sure before posting that your card includes all Platinum/PSN's you want to list.
  If you need more information about trophy cards in signatures, then read: WBA's thread
  Only cards that show us your details work, so EU-PSN cards will NOT be accepted.
  If using , go to your settings, change Who can see your games to everyone and hit save.
  
• Site Policy: All trophies must have been earned fairly and not through hacked or bought game saves.
  Submitting cheated games will result in a permanent ban from us, and we will report you. This only applies to games you listed for your card.
• Only request a card/avatar if you plan to use it as your avatar/signature (spoiler is OK). If you don't use it actively, we will instead use our time and energy for those who do.
  (if it has been a long time between updates and your card is drastically out of date, then it is ok if you remove it from your signature. You will not be banned from the thread if you use it for a reasonable period, forget about it for a while and then come back, or anything to do with designers taking a long time to update).

General Info:

• Be patient! This takes a lot of time to make (we need to verify you earned them and then get the image for each and every game, before even working on the card itself), so expect to be waiting till we have time. Repeated "where's my card" posts are not tolerated. Any rudeness towards the designers will result in an immediate ban.
• If you don't like the card made for you, don't be scared to tell the designer what you'd like updated/changed.
• If you have an un-filled new/update request, and earn more / before it is done, just add the new ones into your existing request. Do NOT a make new post saying you earned a new platinum/100%.
• We do NOT sort/group by game name/type/etc. New games will be added to the NEXT slots on your card.
• DO NOT request an update every bronze trophy for the cards which display full trophy numbers. Only request an update when you have a new image to add, or desperately need something changed on the card.

You MUST follow the rules listed in this post and those above it. If you have a card from a bad source (such as EU PSN), do not list the games, ask for an update and don't have our card in your sig, etc: you will be ignored.
If you list a game that we find you hacked, you will be banned forever.

Avatar: (Optional. No avatar for PSN/Mini cards. Link directly to the image you want beside your name. Image should be same height and width, such as 20x20. If larger, it will be scaled down to size. NOTE: large images scaled down usually do not look good. Skip for updates with same avatar.)
