Can't unlock insane Difficulty!!

I have beaten HARD 3 times now!! & I got NOTHING! What am I doing wrong!?

You have to unlocked and beat all the bonus levels .

No problem. But on post #2 of the guide, under INSANE DIFFICULTY CLEAR, it says to beat all the bonus stages first. Eternal_Spoon found this out and added the info in post #9.
