We would know from the time stamps, the one thing you can't alter. When you play online, the time stamps are from the time that is downloaded from the internet. The game saves are set up to unlock multiple trophies at once. If someone has many trophies unlocked at the same time, that shouldn't have been, we'll know.

As for the "You did not earn a trophy message", that doesn't show for some games. For others that it would, the saves are hacked.
