Heavy Medals

Do I have to get these medals by playing only on hard or can they be done on easy difficulty after the hard playthrough is done?

If I have to replay this game on hard once again I give up on that plat.. only for in game achievements, I got one silver medal on a mission on hard.. full health 87% accuracy.. I don't understand why, is time involved too?
