Do you have to start campaign all over for Insane-trophy

Or can I just beat all the stages, using chapter select?
The reason I'm asking is, because when I start new campaign, I lose all the EGO boost...
I tried to find the answer on this site, but couldn't find.

I haven't gotten that far, but I'm pretty sure I've read it somewhere on this forum; you should be able to get the trophy by using chapter select.

As long as you get the red Nukem logo in the bottom left on every level.

Yes, you can use chapter select for any of the difficulty related trophies.
