For the trophy for finding axel, does it matter if I already have him unlocked from doing side missions?

Nope, you can find his room with or without axel in your team.

Do I need a trap with a rarity of 0 or would a rarity of 1 be ok? I found a rarity 1 trap on one of the dlc characters and would love it if I could use that one.

If you are going for the Magichange option a 3xRarity 1 Trap will give you a +20% boost on your stats, instead of 30% (since the magichanged weapon will be r0) Anyways just stick with said Trap till you meet the 'real' Baal and you will be able to steal a r0 trap with no problem

EDIT: Also for Puzzle Master you might want to mention that you need to throw the blue Geo Block (which is next to the base portal) onto any red panel and destroy it.
