Do you have to complete the game on hard cause in the brackets it says "also unlocks for medium difficulty". But i have completed it on medium and i only got the Tempus Fugit trophy.

I am sorry but you hav missunderstood, re-read it

Complete all campaigns on hard difficulty level (also unlocks trophy for medium difficulty)

Clearly states all campaigns must be played on hard difficulty and doing so will give you the medium difficulty trophy also.

Yea sorry dude, you misread it. It makes sense that beating the game on hard will give you the trophy for Medium as well as Hard. But not the other way around.
