From my personal experience however I can say that it's just you and your direct partner that shouldn't die. I remember that when I got the trophy one of the other two guys died very close to the end of the first map and I was like "damn, all the good work for nothing". But after we won the round the trophy suddenly popped for me and my partner (don't know about the other two since they were randoms). |

Yeah, all 4 of you have to make it. If one of you dies, then you'll have to wait till the next map and get lucky. One whole map being 4 mini-levels (or rounds). You'll know when you're done with a map when Alice says you're done, the helicopter is approaching for extraction, etc.
It's -- obviously -- a good idea to get this done with on the first map, because the deeper you go into Extraction, the harder it will get. Just be aware of the heavy armor guys (RPG, grenadier, etc.) and stay close (but not bunched) to each other. Communicate well, stay close, and make sure to be around each other to revive each other in case, but not close enough to the point where an RPG guy can knock you all out with one RPG (which can happen lol). Extraction is easier than it sounds, but Bound for Life will get your heart pumping lol. But after you ding BfL, Extraction is a breeze. Quite fun, too (just don't play it with n00bs or randoms). ~ Rafdog89
