Counts as "damage." Instead use Flesh Mistress as you take no damage during Hysteria. That is how I obtained the Pulling His Strings trophy.

Does this mean that if you use the Flesh Mistress outfit you can get hit, but you get the trophy nonetheless?

Or is there something else to this?

U don't need the DLC. The final boss is very simple, I got this trophy on nightmare and without the DLC.
