I had the same problems, your issue is your saving the girl, you MUST disarm the bombs. you need at lest +9 rep before you enter the bomb room. it may be lower, but I've not seen any information on that yet.
With 100% intelligence, and 10 rep, I did it both ways, it won't work if you save her, she must die.
I had no contact with SIE yet. I am not sure if it matters to have mostly professional throughout your game to that point.
if you see an option that says something like "so what, I just let you go?" then you will get it, it's the last option in conversation before he runs and space monsters eat his face (DLC only event)
