Thanks for the info, but I'll have to verify it as the trophy itself is glitched in several ways, not just the way I detailed it. A few people (including myself) have completed the game from start to finish with the same companion and not gotten it, while others such as yourself have gotten it just by replaying the last 2 levels. Judging by the results so far, it may be tied in directly with when or if your companion would have gotten the trophy.
