Would You Like Fries with That?
Landed 100 complete attack chains.

Simply land 100 chains of attack. Every weapon will have a sequence of attacks which you can combo by pressing or repeatedly, all will be fairly obvious when the combo has ended, as there will be a significant pause before the next attack. Have 100 full combos strike enemies and this trophy will pop. This will come in the course of normal gameplay.

Shock and Awe
Killed 100 enemies with abilities.

See And Then There Were None.

And Then There Were None
Killed 500 enemies with abilities.

Defeat 500 enemies with a magic derived ability ( + ///). You only have to do the final blow with a magical ability, so whittling down an enemy with basic attacks, then using magic to finish it off is an effective way of farming this trophy. If you are having trouble finishing off this trophy, head back to the first areas and use abilities to one-hit-kill all enemies you encounter. Area of effect abilities are especially effective for farming with this method.

Juggler
Landed 5 consecutive hits on a launched enemy.

There are several ways to accomplish this. In the Might tree, you can use Wrath to launch an enemy, in the Finesse tree you can use Gambit to launch an enemy. Then just hit the enemy as many times as possible before it lands. You can also simply do the method right at the start of the game where you use a longsword to attack, then pause, then attack again. This should launch the enemy into the air. Then pull out a ranged weapon, for instance, the bow that you start off with, and shoot as fast as possible. You should be able to land at least 5 hits before they touch the ground.

Elixer of Fate
Made a potion with the Essence of Fate.

You will earn this for creating a potion which has at least one component as 'Essence of Fate'. The one I used was a Fate Potion. It should be listed in the ‘Create’ tab at the alchemist; I didn’t have to do any experimenting to unlock it. The fate potion is created by combining 4 Essence of Fate.

So imo the easiest way to get this trophy is leveling your Alchemy to 7 (not sure if that's the correct number, but up to the point when you can use Essences of Fate) and learning how to make a Master Healing Potion via experimentation combining 3x Black Cohosh + 2x Embreyes + 1x Bloodroot + 1x Essence of Fate. After you learn the recipe go to the "Create" tab and make the potion from there, and the trophy will pop.
It Didn’t Explode!
Made a stable potion by experimenting.

Simply experiment with recipes until you find a proper potion recipe. I suggest creating a Minor Healing Potion, as this is a simple recipe that will be useful, especially early. This recipe is created by combining:
2x Black Cohosh + 1x Embereyes.
Note that though it requires 2 Black Cohosh per use, the Black Cohosh only takes up one slot in the experiment, so you will only need two total slots (which you start with) to create this potion. Do not worry about specifying quantity, it will automatically adjust for you after you complete the experiment.

Green Thumb
Harvested 10 of each type of reagent.

There are 19 different types of reagents. They are as follows:

You can collect a reagent by walking up to it and clicking . You will not always receive a reagent when you attempt to harvest one, but you can increase your chances of being successful by putting more points into Alchemy. Reagents respawn quite often, so though this trophy is a giant pain, it is not missable.

A great way to farm more than half of the reagents is at Seawatch castle at the middle-right of the Caeled Coast. You do a quest to restore it and there is a ton of reagents there. Farm them, sleep for 2 days, repeat.

Repaired a piece of equipment.

There is two ways you can repair your equipment. The first is to go to a blacksmith, which you can find in most towns, talk to the blacksmith, and ask them to repair your equipment for a fee. The second way is to find a repair kit and use it. Repair kits can be used by going into the menu, selecting Inventory, Items, finding Repair Kit, and selecting use. Then simply select the item you would like to repair.

Shop Class
Crafted a piece of equipment with Blacksmithing.

Simply go to a Blacksmith Forge and create an item. You will need different required materials for each basic creation. You only need to create an item for this trophy, it doesn’t have to have any specific number of components going into it, though the minimum is two.

Master of the Forge
Crafted an item that uses all 5 forge component slots.

To create an item that uses all five forge slots you will need to have Blacksmithing level 7 or higher (which requires you to be level 8). Once you have level 7 Blacksmithing, you will need the components to create an item. You can buy materials from local merchants if you haven’t found enough yet. The last slot, labeled ‘Bonus’ can only be filled by a gem, which can be bought, or created using Sagecrafting. I found that only certain gems would go into certain types of equipment, though it was not clearly labeled which would match up. On the Blacksmith Forge main hub, it will have all of the components in a row when you are choosing what to create. If you have at least one of each component compatible to that item, all of the components will be lit up.

Romancing the Gem
Crafted an Epic Gem.

To create an Epic Gem, you must have Sagecrafting level 8 or higher (which requires you to be level 16). To create an Epic Gem, you will need two Pristine Shards, which you can craft (see Diamond in the Rough), find, or buy. Then simply find a Sagecraft Altar, select Craft Gems, Epic Gems. Then choose any two Pristine Shards (you can use the same type) and combine them for this trophy.

Diamond in the Rough
Crafted a Pristine Shard.

To craft a Pristine Shard, you will need at least level 3 Sagecraft. You will need two Lambent Shards. These can be obtained by finding them, buying them, or crafting them by combining two Cloudy Shards. When you have two Lambent Shards and level 3 Sagecrafting, go to a Sagecraft Altar and select Craft Gems, Strengthen Gems. Then choose any two Lambent Shards (you can use the same type) and combine them for this trophy.
